The applicant must provide the following information: a. Location of work – Work will be at in the PUE north of 139 North Dobson, Mesa, AZ. This will shut the drive down at Arizona College. College has been notified that we will be working in the drive. b. Schedule of hours and days – Work will be on weekends – 1/14-1/15, 1/21-1/22, 1/28-1/29/2023. We will need the extended hours permit to cover work during the day and night on weekends per Inspector Dale Brunk. c. Scope of work to be performed – Water line tie in from ROW to middle of the street. d. Anticipated noise generated from the work performed – Construction noise generated from digging a trench. e. Construction permit number (ex. Pmt, utl, row) – pmt21-14923 2. Applicant shall notify all residence within 500 feet of the project property line in writing. f. The notice shall explain the purpose of the afterhours work and the times and days the work will be performed. Flyers to be provided to all residences within 500 feet of project property line. See attached sample. g. The notice shall provide the name and phone number of the after-hours work supervisor. Austin Miceli contact information will be on all flyers. 3. All reasonable efforts will be taken to mitigate noise. 4. All reasonable efforts will be taken to mitigate fugitive light. 5. The applicant will respond positively to neighbor complaints. 6. The Development Services Department can cancel this permit if legitimate complaints are not resolved. 1/10/23 Construction Notice Please be advised we will be doing water line work in the ROW and street on 1/14 thru 1/15.
